 A few questions on PayPal
Do I need to do the sandbox test? I don't really know what this is our why is important?
I'm in New Zealand, do I need a currency conversion?
I followed the steps (apart from the sandbox things) but when booking if I click PayPal card payment, nothing comes up.

Do I need to do the sandbox test? I don't really know what this is our why is important? - no, you don't need to use Sandbox, it's just for testing and we have already done all testing, all you have to do is enter your PayPal address and set Sandbox: No
I'm in New Zealand, do I need a currency conversion? - I don't think you need currency conversion as New Zealand $ is supported by PayPal, currency conversion is only for unsupported currencies by PayPal and will convert the website currency to one that is supported by PayPal so the transaction can be done.
I followed the steps (apart from the sandbox things) but when booking if I click PayPal card payment, nothing comes up. - I have tested from test@test.com and was correctly redirected to PayPal when I selected PayPal/Credit and Debit cards. There is a large blue button that the customer has to click on to confirm PayPal redirection to process the payment "For security reasons please confirm that you would like to pay via PayPal by clicking here.".
