Hi Lawrence,
this has been fixed on your booking form and it's now working as expected.
Please clear your browser's cache by holding the Shift button and clicking refresh on your browser or from the browser settings menu.
After you clear the cache you will see that the 3rd drop down to select Drop off category is working as expected.
